Forms

There are two options for building and managing forms in the website.

Jotform

Jotform is the university’s secure form solution.

Access to Jotform is based on individual WUSTL Key. To get a user license for Jotform, contact IT through ServiceNow.

Formidable Forms

You can find existing or create new forms by clicking on the Formidable plugin in the left hand menu of the dashboard.
